Our second of several Questionnaires from DBT Holistic Skills Workbook for Teens, this Meal Reaction Questionnaire (Page 163) Printable Handout is the perfect way for students to check in about whether or not any particular meal helps them feel better or worse!

This questionnaire is meant to be taken approximately 30-90 minutes following any particular meal.

The objective is to watch as students end up circling more and more “Yes’s” with each nutritional experiment they implement. Will they try eating more protein for a few weeks? Maybe see how veganism affects their bodies? Or maybe see how they feel after a few weeks of avoiding processed foods!?

These are the kinds of questions that students can easily explore with 20 quick questions, answering Yes, No, or Unsure.

Whatever the results, they will be on their learning journey with nutrition!
